In June of this year, we received a call from a discharge planner at Boca Raton Community Hospital regarding Mrs. H, a 64-year-old woman living in Century Village. One of our Patient Care Advocates went that day to meet Mrs. H and assess her needs for home care assistance.
Upon meeting Mrs. H, our team member immediately recognized the signs of cognitive impairment and the need for a more comprehensive care plan. Unbeknownst to the hospital discharge planner, Mrs. H had suffered a stroke six years ago with delayed cognitive effects. She also had diabetes, a seizure disorder, low sodium levels, and a history of falls — none of which had been properly monitored.
To address these concerns, Boca Home Care Services arranged for a live-in aide and appropriate medical equipment for Mrs. H. A pair of talented and devoted home health aides worked together with a registered nurse to coordinate the necessary care. Physical therapists, occupational therapists, and a medical social worker from our sister company visited Mrs. H at home, providing continuous services to ensure her safety and improve her overall well-being.
We also contacted Mrs. H’s daughters, who were unaware of the severity and complexity of their mother’s condition. One daughter immediately planned a visit and met with us to discuss their mother’s present and future care needs.
