Trust Us to Be Your Loved One’s Advocate

As we age, we accumulate numerous points of care: a primary care physician, a cardiologist, a neurologist, a pharmacy, a physical therapist; the list goes on…

A Geriatric Care Manager or Geriatric Care Coordinator can help seniors plan and coordinate all of these point of care. They can also help clients select new providers as their needs change.

GCMs can also attend appointments with clients to ask questions, take notes and make sure that specialists are getting all of the information they need to provide appropriate holistic care.

Boca Home Care Services has licensed social workers on staff who can provide basic Care Management services. We can also refer more complex cases to independent GCMs with specialized practices.

Let us help you keep an eye on your loved one’s care. We’re here 24/7, available on call even when you can’t be.

  • Install safety bars around toilets and in the shower; connect a handheld showerhead and place a seat in the shower
  • Place a safety mat or non-slip strips on stairs
  • If your loved one is living alone install an emergency response system
  • Ensure good and easily-turned-on lighting in the kitchen, bedroom and bathroom, including nightlights in the bathroom and hallways
